Subject: Educational Assistance (Student Fee Discount) for Spouses and Dependent Children of Employees

The form for requesting the fee discount for spouse or dependent child is available at the link below or in the Human Resources Office. After completion of Part I and II, the Human Resources Office shall verify and sign Part III, retaining a copy for personnel records. The employer certification signature should be obtained no earlier than 30 days before the fee payment deadline for the semester. Signatures may be obtained and submitted beginning July 1 for Fall semester, November 1 for Spring, and April 1 for summer.
